PITFALL 6
Expecting staff to document adaptation without tools and support
WHY IT'S A PROBLEM
Staff are often focused on implementation and may feel unprepared to monitor activities. They assume they’ll remember key learnings but without structured documentation, important insights are lost and adaptation quality suffers.

LEARNING
Use simple tools to monitor adaptations in real time
HOW THE PITFALL WAS OVERCOME
In the Democratic Republic of Congo, efforts to strengthen quality VAW prevention programming were undermined when staff and community facilitators did not consistently monitor or document changes. Without this information, it was difficult to understand what was adapted, why, and with what results. Developing a simple, structured documentation tool and reallocating budgets for a dedicated staff member focused on documentation improved programme quality, strengthened institutional memory, and supported sustainable adaptation.
